In this role, you will conduct health assessments, advocate for patient engagement, and monitor treatment effectiveness for those with complex health needs. Key Responsibilities:

• Conduct health assessments for patient triage

• Advocate for patient participation in care plans

• Monitor treatment plans and their impacts

• Facilitate communication with community care providers

• Deliver emergency and acute care interventions

Requirements:

• Must be a Registered Nurse in good standing

• Experience with complex health care needs

• Strong critical thinking and organizational skills

• Excellent communication and interpersonal abilities

• Willingness to work rotating shifts in a 24/7 facility
