Nurse Practitioner in Primary Care Services

Location: greater sudbury

Enhance health outcomes as a Nurse Practitioner at Shkagamik-Kwe Health Centre. Focus on comprehensive care, chronic disease management, and culturally informed practices.
In this full-time role based in Sudbury, ON, you will deliver health care services across various age groups. The position emphasizes teamwork within a multidisciplinary framework, integrating Indigenous healing approaches. You will manage patient health and provide education to clients and the community.

Key Responsibilities:

• Offer primary care including assessments, diagnosis, and treatment

• Collaborate with clients on chronic disease interventions

• Conduct procedures and diagnostic testing within scope

• Coordinate mental health support and referrals

• Engage in community health initiatives and workshops
Requirements:

• Master’s degree in Nursing or recognized NP program

• Valid registration with College of Nurses of Ontario

• Minimum 2 years of NP experience preferred

• Strong clinical and interpersonal skills

• Familiarity with Indigenous health initiatives is beneficial
