Location: Northeastern Manitoulin and the Islands
Lead diabetes education initiatives as a Registered Nurse and Certified Diabetes Educator within a dedicated wellness team. This role involves providing essential self-management education to enhance client health outcomes.
As part of the Diabetes Wellness Team, you will perform assessments, implement care plans, and educate clients and families about diabetes management. Your position involves significant collaboration with health professionals, ensuring comprehensive care while addressing chronic illness prevention and management strategies in diverse communities.
Key Responsibilities:
• Provide education and support for diabetes self-management
• Implement and assess client care plans regularly
• Collaborate with dietitians for holistic health evaluations
• Execute medication reviews and recommendations
• Contribute to organizational quality improvements and reports
Requirements:
• Bachelor's degree in Nursing
• 1-3 years of clinical/community experience
• Good standing registration with the CNO
• Diabetes education experience is beneficial
• Valid Class G driver’s license with clear check
