Position: Full-Time Nurse Practitioner at Extendicare
Location: Northeastern Manitoulin and the Islands

Extendicare seeks a dedicated Nurse Practitioner for comprehensive primary care. This full-time contract role involves direct patient interaction and interdisciplinary collaboration.

As part of the Extendicare team, the Nurse Practitioner will provide essential care to residents, ensuring adherence to the Resident Bill of Rights and long-term care regulations. With a focus on health assessments and care management, candidates with 3-5 years of clinical experience will excel. Forge strong connections with residents, families, and healthcare professionals in this pivotal role.

Key Responsibilities:

• Provide comprehensive primary care to residents

• Conduct health assessments at resident admission

• Manage care through pharmacological and counseling interventions

• Collaborate with the interdisciplinary team on care plans

• Lead quality improvement initiatives and staff coaching

Requirements:

• Master’s of Nursing or equivalent degree

• Completion of NP Field Diploma

• Registration with the College of Nurses of Ontario

• 3-5 years of clinical experience in Long-term Care

• Excellent communication and problem-solving skills
