Registered Practical Nurse Long Term Care

Deliver person-centred nursing care as a Registered Practical Nurse with Extendicare, using clinical judgment, care coordination, and medication administration.
Reporting to the Director of Care or Director of Health and Wellness, you will provide planned, patient-oriented, goal-directed care. You will assess residents, maintain care plans, coordinate services, and document clinical information according to provincial standards. The role supports residents in a long-term care environment while collaborating with physicians, families, nursing colleagues, interdisciplinary teams, and contracted providers.

Key Responsibilities:

Provide direct resident care and administer medications and treatments
Complete assessments, care plans, service plans, and documentation
Evaluate care effectiveness and communicate required changes
Coordinate resident care assignments and care conferences
Manage significant condition changes and medical emergencies
Requirements:
Current registration as a Registered Practical Nurse
Strong clinical knowledge and critical thinking skills
Current Basic Cardiac Life Support and First Aid certification
Computer proficiency with Microsoft Office applications
Current Vulnerable Position or Sector Screening within six months
