Position: Social Worker for Community Health Services
Location: Greater Sudbury

Make a difference as a Social Worker at Shkagamik-Kwe Health Centre in Sudbury, Ontario. This full-time position is committed to delivering holistic services and supporting diverse populations through culturally safe practices.

This role requires a Registered Social Worker with a Bachelor or Master of Social Work, with at least two years of experience in community health. You will assess client needs, implement care plans, and provide necessary support while ensuring advocacy and collaboration with multidisciplinary teams. Your efforts will help foster resilience and empower clients within their communities.

Key Responsibilities:

• Evaluate clients’ social and emotional needs

• Create personalized care plans with clients

• Manage cases and provide crisis intervention

• Offer group support and therapeutic interventions

• Collaborate with community resources

Requirements:

• BSW/MSW from accredited institution

• Registered with OASW or equivalent

• Minimum two years’ experience in social work

• Familiarity with Indigenous community practices

• High level of cultural competency
