Stern Therapy Consultants is seeking a PRN Physical Therapist at Bear Mountain in Sudbury, MA
. Work with skilled nursing residents who need individualized rehabilitation to regain movement, manage functional limitations, and participate more safely in their daily routines.
Assess residents' mobility, strength, range of motion, and balance to identify treatment priorities.
Create plans of care with functional goals that reflect each resident's abilities, precautions, and recovery needs.
Provide exercise, gait training, and transfer interventions that build skills for everyday movement.
Recommend appropriate assistive devices and teach residents and caregivers how to use them safely.
Monitor progress, revise treatment plans, and supervise PTA services.
Document evaluations and treatment sessions while coordinating care and discharge recommendations with the interdisciplinary team.
$60–$65 per hour.
PRN assignments coordinated with your availability and facility coverage needs.
Unlimited referral bonus, with up to $1,000 per successful referral.
Access to therapy leadership and collaboration with fellow rehabilitation professionals.
Active Massachusetts Physical Therapist license.
Strong clinical assessment, treatment planning, and documentation skills.
Ability to communicate clearly with residents, families, and care team members.
Skilled nursing rehabilitation experience preferred; new graduates are welcome to apply.
