Your Role & Responsibilities as a Physical Therapist (PT):

• Assess & Treat Patients – Help restore mobility and improve quality of life.
• Create & Implement Treatment Plans – Tailored to each patient’s abilities.
• Set Realistic & Achievable Goals – Help patients hit their milestones!
• Document & Track Progress – Keep detailed records of patient care.
• Collaborate with a Multidisciplinary Team – Because teamwork makes the dream work!
• Educate & Support Patients and Families – Provide guidance for long-term success.
• Ensure Compliance – Uphold HIPAA, confidentiality, and resident rights.

What You Bring to the Team:

Degree from an Accredited Physical Therapy Program

Valid Professional License in the State of Practice

Experience in Long-Term or Geriatric Care Preferred (But Not Required!)

Familiarity with Case Mix Index & PPS Scheduling is a Plus!
