Aging in Place Assistance Program, you will play a vital role in connecting seniors to life-changing resources, building a network of service providers, and ensuring the program’s success through outreach, registration, and education efforts.

Aging in Place Assistance Program, you will play a vital role in connecting seniors to life-changing resources, building a network of service providers, and ensuring the program’s success through outreach, registration, and education efforts.
Prospective Vendor Outreach and Network Building:
- Conduct outreach to licensed Occupational Therapists (OTs) and contractors to build a database of professionals interested in contracting with the Home Safe program.
- Cold call, email, and engage potential service providers to gauge interest and qualifications.
- Maintain an organized database of interested vendors, ensuring all licensing and credential requirements are documented.
In-Home Registration and Application Processing:
- Conduct physical in-home visits to assist seniors with application completion and eligibility verification for the Home Safe program.
- Gather and organize required verification documents, such as income and age qualifications, to ensure compliance with HUD guidelines.
- Provide clear guidance to seniors and their families throughout the application process.
- Conduct door-to-door outreach in targeted communities to identify and engage eligible seniors for program participation.
- Educate seniors about the benefits of the Home Safe program and assist with initial registration steps.
Community Event Outreach and
Education:
- Represent the Home Safe program at community events, senior expos, and health fairs to promote awareness and participation.
- Distribute HUD-compliant educational materials, including multilingual resources, to underserved populations and individuals with Limited English Proficiency (LEP).
- Collaborate with local organizations, such as Area Agencies on Aging, to maximize outreach and impact.
Compliance and Reporting:
- Ensure all activities adhere to HUD’s 24 CFR Part 58 requirements and other relevant regulations.
- Contribute to biweekly progress reports detailing outreach efforts, client registrations, vendor recruitment, and program outcomes.
A Compassionate and Organized Professional
We’re looking for a Home Specialist who brings:
High school diploma or equivalent (Bachelor’s degree preferred).
Experience in outreach, social work, customer service, or vendor sourcing is a plus.
Skills:
- Strong organizational and time management abilities.
- Excellent communication and interpersonal skills, especially when engaging with seniors and underserved populations.
- Proficiency in data entry, maintaining accurate records, and building professional networks.
