** What you'll need:**
* ** Active LPN or RN License issued by NYS**
* ** Minimum of 1 year experience in a home care setting.**
* ** Current CPR/BLS Certification**
* ** Empathy, Reliability , and a commitment to high-quality care
**** What you'll do :**
* ** Perform 1:1 patient care duties and treatments as per care plan.**
* ** Provide health promotion, counseling, and education.**
* ** Administer medications and other personalized interventions.**
* ** Coordinate care, in collaboration with a wide array of health care professionals.**
* ** Document nursing notes regarding patient care daily.**
* ** Must be able to lift 50lbs.**
* ** Must be able to stand/work for extended periods**
* ** Additional responsibilities as required.
