Responsibilities

What You'll Do:

During visits that can last up to one hour, you will:

  • Conduct comprehensive assessments, including medical history, diagnosis, treatment, health education, and referrals.
  • Administer and order necessary point-of-care and lab-based testing.
  • Collaborate with physicians, case managers, social workers, and caregivers.
  • Provide services at the member’s residence, whether at home or in Skilled Nursing Facilities.

Qualifications

Skills & Experience That Shine:

  • Master’s Degree or equivalent experience with NP or PA licensure.
  • Current NP or PA licensure in your state, including prescription authority.
  • Board certification in relevant specialties.
  • Current BLS, ACLS, or CPR certification.
  • 1 year of experience preferred; new graduates are encouraged to apply!
  • Valid state driver's license and proof of adequate automobile insurance.
  • Strong computer skills and familiarity with medical record software.
  • Excellent verbal and written communication skills.
