This role is essential in delivering one-on-one, patient-focused care while maintaining the highest standards of nursing practice.

• Skilled Nursing Care:
  Provide services such as medication management, wound care, and respiratory support in patients' homes.
• Patient
  
  Education:
  
  Educate patients and their families on care plans and health management.
• Documentation and Compliance:
  Maintain accurate records and adhere to all regulatory standards.
• Collaborative Care:
  Work closely with RNs, medical staff, and families to ensure seamless care delivery.

• Qualifications:
  
  Current LPN license in Virginia, with graduation from an accredited nursing program.
• Experience:
  
  At least 2 years of experience in home health care, skilled care, or a related field.
• Transportation:
  Reliable transportation, a valid Virginia driver’s license, and car insurance.

  Familiarity with home health equipment and patient care documentation.
