Hospice - PRN - Registered Nurse (RN) role at Frontpoint Health — Sugarland, TX 77477

As a Hospice Nurse or RN, you will be responsible for traveling to several homes a day to administer healthcare services and help patients maintain their independence. Duties may include administering at home IVs, changing dressings, cleaning wounds, and foley/catheter insertions.

• Case management and supervised visits.
• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Assess and chart observations of the patient’s condition at each visit (vital signs).
• Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing the healing progress).
• Provide proper patient education for home health care procedures and strategies.
• Responsible for the overall clinical record maintenance to include visit note (must be completed within 24 hours), comprehensive assessments, including OASIS (must be completed within 48 hours), medication profile, orders/plan submitted in a timely manner, etc.
• Maintain confidentiality of patient PHI through HIPPA law.
• Keep the patient comfortable through palliative care.

• Current Texas State license as a Registered Nurse.
• 2+ years experience as a RN in Home Health.
• Current CPR certification.
• Reliable transportation.
• Texas Drivers License and Auto Liability Insurance.
• Current TB Skin Test
• EMR experience (Kinsser preferred).
