Overview

Clarius Medical Group, PLLC is excited to announce a new opportunity for a dedicated and dynamic Nurse Practitioner to join our team full-time, focusing on our Clear Lake facility with additional coverage throughout the Houston area. This position involves in-person rounds and is integral to offering superior patient care across our network. As the primary NP for the Clear Lake facility, you will play a crucial role in providing high-quality medical care and enhancing patients’ continuity of care within a structured healthcare environment.

The role requires routine travel to the Clear Lake facility and occasional coverage at other partner facilities in the Houston area.

Job Description

The Nurse Practitioner at Clear Lake Facility will serve as the primary medical provider responsible for conducting patient rounds and ensuring high standards of care. This role is ideal for someone who thrives on making a meaningful impact on patients’ lives through dedicated and empathetic care in a community setting. The position is based at the Clear Lake facility and may require travel.

Duties

and Responsibilities
  • Serve as the primary rounding provider at the Clear Lake facility, ensuring consistent and comprehensive patient care.
  • Perform patient evaluations, manage ongoing treatment, and respond promptly to acute changes in patient conditions.
  • Collaborate with multidisciplinary team members including nursing staff, social workers, and case managers to optimize patient outcomes.
  • Administer medications and manage medication plans, including reconciliation and reviews, as per established protocols.
  • Maintain detailed patient records, ensuring all documentation is accurate, timely, and compliant with legal and regulatory standards.
  • Communicate effectively with supervising physicians and other healthcare providers to discuss care plans and patient progress.
  • Provide occasional support and coverage for other facilities within the network, ensuring a consistent level of care across all locations.
Requirements
  • Must possess an active Texas APRN/NP license in good standing.
  • Previous experience in skilled nursing facilities, long-term care, group homes, hospital-based rounding, or home-based care is preferred.
  • Demonstrated ability to operate independently with minimal supervision in a healthcare facility environment.
  • Strong clinical judgment and critical thinking skills, capable of making swift and sound decisions.
  • Exceptional communication skills, both written and verbal, and proficiency in electronic health record systems; eClinical

    Works preferred.
  • Must have reliable transportation and be willing to travel to the Clear Lake facility as the primary work location.
Schedule / Expectations

This in-person role is designed around the needs of the facility rounding schedules, with additional coverage responsibilities as required.
