Registered Nurse – Home Health

This position involves providing specialized nursing care through home visits, assessing patient needs, developing personalized care plans, and collaborating with an interprofessional team to ensure quality outcomes. Responsibilities include evaluating patient preferences, promoting culturally sensitive care, and updating care plans based on patient condition changes. • Minimum of a Nursing Diploma/Associate’s degree in Nursing
• Valid RN license and driver’s license
• At least 2 years of nursing experience; preferred Bachelor’s degree in Nursing or Home Health with 2-5 years of experience
• Strong critical thinking, communication, and cultural competency skills
• Ability to work safely and efficiently in a safety-sensitive environment

• Assess patient needs and barriers to care, including emotional, spiritual, and cultural factors
• Develop, implement, and document individualized care plans with measurable goals
• Coordinate care across the continuum, ensuring effective communication and collaboration
• Monitor patient condition changes, update care plans, and communicate with caregivers and the team
• Promote respectful, equitable, and empathetic interactions with diverse populations
