JOB DESCRIPTION

Atlantic Health is seeking two (2) skilled and compassionate per diem outpatient Physical Therapists to join our team in our Clark, NJ location, three (3) days per week (M, W from 11a-7p, and F 9a-5p).

(1) Per Diem for general Ortho

(1) Per Diem for Lymph - CLT/CDT is required

• Conduct comprehensive evaluations and develop patient-centered treatment plans
• Collaborate with physicians, and fellow rehab team members
• Communicate with patients and families to ensure understanding and continuity of care
• Support various appointment types and adapt care plans as needed

Essential functions include patient evaluation, establishing plans of care and treatment programs, providing interventions, providing patient and family education, documenting interventions, and billing appropriately. Supports Lead Clinicians in competency development and implementation. Participates in the orientation of new staff members, serves as a clinical instructor for students, exercises independent judgement of moderately complex cases and with minimal to no supervision.
