Full Time, Day Shift. This position is for Sumter/Clarendon.
Job DescriptionLicensed Practical Nurse in home health setting responsible for delivering high quality, patient‑centered care. You will collaborate with an interdisciplinary care team and work under the direction of the primary RN.
Essential Functions- Maintain a professional image and exhibit excellent customer relations with patients, visitors, physicians, and co‑workers in accordance with Service Excellence Standards and Core Values.
- Assist with patient revisits within a geographical area, supervised by the Team Clinical Manager. Work under the direction of the primary RN and collaborate with team RN, PT, OT, ST, and MSW for interdisciplinary coordination of patient care.
- Demonstrate strong communication skills, attention to guest relations, and effective interpersonal relationships. Good writing skills and basic computer literacy are desirable.
- Handle safety issues for staff and patients in all settings, applying principles of growth and development over the lifespan. Assess data reflective of the patient’s status and interpret information needed for age‑specific care.
- Exercise high degree of maturity, self‑motivation, creativity, initiative, and independent judgment. Demonstrate responsible, ethical, self‑initiating professional behavior.
- Communicate efficiently with a diverse population, listen to staff and patients, and participate in planning with physicians and external agencies. Maintain a neat, clean, and well‑groomed appearance and adhere to dress code and attendance policy.

Qualifications/Training ul>- Must possess a current valid SC driver’s license and automobile insurance as required by law.
- LPN license and BLS certification required.
- Must meet McLeod policy for driving eligibility and be at least 21 years old to be covered under McLeod Health’s fleet vehicle insurance policy.
- Possession of professional liability insurance is encouraged.
