Responsibilities Delivery of patient care
  • Provide personal care (ADL) including bathing (assist, bed bath, tub bath, shower, sitz), body mechanics (turn, position, transfer, use of lifts, ambulation, range of motion), nutrition (prepare meal, feed patients, measure intake/output), elimination (bedpan/urinal, bowel/bladder retraining, specimen collection, perineal/catheter care, condom catheter, douches, enemas, rectal tubes, drainage, gastric suction), safety (side rails, restraints, CPR/Heimlich, infection control, isolation, universal precautions).
  • Perform special procedures: vital signs (temperature, oral, rectal, axillary; pulse, radial, apical; respiration, BP; height & weight), heat/cold application, decubitus ulcer prevention and care, surgical skin prep, clean dressing changes, apply ace bandages, TEDs, binders, post‑mortem care, assistance with cough/deep breathing.
  • Perform housekeeping services necessary for health and safety of patient, family, caregiver.
  • Participate in interdisciplinary team conferences (IDT) and special case conferences.
If functioning as a NA I
  • Sterile dressing for wounds over 48 hours old; wound irrigation.
  • Ostomy care or ostomy irrigation.
  • Insertion of urinary catheter limited to replacement of an existing catheter after nursing assessment (no irrigation).
  • Oxygen therapy including room set‑up and monitoring of flow‑rate.
If functioning as a NA II
  • Breaking‑up and removal of fecal impaction.
  • IV fluid‑assisted activities (assemble/flush tubing during set‑up, monitor flow‑rate, site care/dressing change, discontinue peripheral intravenous infusions).
  • Nutritional activities (oral/nasogastric feedings after placement verification, gastrostomy feedings, clamp tubes, remove oral/nasogastric feeding tubes).
  • Suctioning (oropharyngeal, nasopharyngeal).
  • Tracheostomy care.
  • Insertion of urinary catheter limited to replacement of an existing catheter after nursing assessment.
  • Sterile dressing for wounds over 48 hours old; wound irrigation.
  • Ostomy care or ostomy irrigation.
  • Oxygen therapy including room set‑up and monitoring of flow‑rate.
  • Interdisciplinary team participation and establishing effective working relationships.
  • Documentation: accurate, complete, succinct, and timely clinical documentation in accordance with HPCCR guidelines and licensure requirements.
  • Professional and personal accountability: competency, maintaining professional reputation, ongoing professional development, CPR certification, CEU requirements, external agency relationships, time management, crisis response, additional duties as required.
  • Cleaning patient rooms and facility areas per procedures, ensuring supplies, using protective equipment.
Qualifications

Minimum 1 year experience required. Experience in home care, working with terminally ill and/or geriatrics preferred.
