Overview

LVN/LPN (Healthcare)

Responsibilities
  • Administrative Functions
    :
    Direct the day to day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long term care facility. Ensure compliance with written policies and procedures. Review and revise department policies and manuals as needed. Plan shifts, services, programs, and activities with staff and support personnel. Maintain the Nursing Service Procedures Manual and ensure all nursing service personnel comply with its procedures.

    Prepare written and oral reports on shift activities. Coordinate nursing services with resident services to maintain the resident’s total regimen of care. Participate in the facility’s quality assurance program and facility surveys. Review and update discharge plans and assist in planning nursing services for discharge. Interpret policies to personnel, residents, visitors, and government agencies. Admit, transfer, and discharge residents as required.

    Complete accident/incident reports and maintain census and documentation. Conduct administrative duties such as medical forms, reports, and charting. Protect resident information and restrict access to authorized individuals as required by policy.
  • Charting and Documentation
    :
    Complete and file required recordkeeping forms upon admission, transfer, and discharge. Encourage physicians to review and sign orders and progress notes. Transcribe physician orders and document nursing notes with care details. Complete accident/incident reports and transfer forms as required. Maintain diet orders, communicate with Food Services, and report discrepancies. Ensure documentation of unauthorized discharges as required. Perform routine charting and ensure entries are signed and dated.
  • Drug Administration Functions
    :
    Prepare and administer medications as ordered. Verify resident identity before administration. Manage floor stock and report needs. Order medications and supplies as needed. Maintain narcotic records accurately and report discrepancies. Review medication cards for accuracy and adherence to stop orders. Dispose of drugs per procedures and comply with regulations.
  • Personnel Functions
    :
    Contribute to staffing evaluations and scheduling; identify and report staffing needs. Lead and supervise nursing personnel; perform daily rounds to ensure standards. Ensure CNAs are trained and supervised; oversee discipline and grievances. Foster collaboration with inter-departmental teams and maintain a supportive unit atmosphere. Ensure adherence to policies and procedures, and maintain warmth and a calm environment.
  • Nursing Care Functions
    :
    Inform staff of admissions; prepare rooms; orient new residents and families. Coordinate physician visits, diagnostic and therapeutic services, and restorative programs. Ensure care plans are followed and supervise CNA performance. Manage admissions, transfers, and discharges; arrange transportation for discharges. Escort discharged residents and inform families of deaths where applicable. Carry out professional nursing procedures and ensure patient safety and dignity.

    Use restraints only per policy and ensure re-evaluation of care plans as needed. Monitor residents, especially when seriously ill, and check dietary allowances for residents’ meals.
  • Staff Development
    :
    Develop and deliver in-service training; provide orientation and onboarding for new hires. Support ongoing education, OSHA, HIPAA, safety, infection control, and other required trainings. Participate in external training and facility in-service programs to maintain current licensure and knowledge.
  • Safety and Sanitation
    :
    Enforce safety regulations, dress codes, and infection control procedures. Ensure hand hygiene, use of PPE, and safety equipment. Participate in fire safety and disaster drills.…
