We are a forward looking training practice committed to offering high quality patient care whilst providing a friendly,supportive and enjoyable place to work. We are in leafy Surrey, a stones throwfrom the River Thames with a patient population of 19,300.
We are looking to recruit a caring and dedicated practice nurse to join our nursing team comprising a Nurse Manager, 3 Practice Nurses, Nursing Associate, HCA & 3 Phlebotomists.
Previous GP Practice and chronic disease management experience, particularly in Asthma, is desirable although training is also available.
Emis Web clinical system experience would be an advantage.
Main duties of the jobTo deliver high quality integrated nursing care to the practice population and promote health and well being.
To work with clinical and non-clinical practice staff and other members of the wider healthcare team contributing to the achievement of QoF targets.

GPs, prescribing ANPs, Paramedics & Clinical Pharmacists, Pharmacy Technician, Practice Nurses,HCAs, Phlebotomists & Care Coordinators.
With a list size of 19,300patients we are a training practice rated Good in all areas by CQC, high QOFAchievers with strong links to the ICB and local GP Federation.
Job responsibilities- To provide assessment, treatment, screening and health education services and advice
- Provide nursing service/treatments to patients independently or in participation with general practitioners with reference to agreed protocols and guidelines.
- To work on own initiative whilst recognising when referral to other health professionals is needed.
- To be familiar with changes and progress in nursing care.
Nursing procedures to include, but not limited to -
- Measuring blood pressure
- Routine injections as prescribed
- Cervical Smears
- Childhood and Adult immunisations, including reading and reviewing Patient Group Directives (PGDs) and Patient Specific Directives (PSDs)
- Well Person Health Checks including Foreign Travel Counselling, NHS Health Checks, Lifestyle and New Patient Checks
- ECGs
- Ear microsuction and assessment of ears
- Obesity and weight/diet management
- Family Planning including assisting in Implants & Coil fittings & Sexual health
- Chronic disease management and prevention
- CVD risk assessments and Q risk interpretation and assessments.
- Occasional home visits for housebound patients.
- Referrals Podiatry, Counselling, Audiology, etc
- Ensuring all specimens are checked and sent to laboratory as required
- To be fully conversant in QOF, read coding and uphold strategies to provide a holistic combined healthcare for the patient.
- To be responsible for the efficient running of the treatment room and equipment and to ensure the clinical areas are at all times safe for staff and patients.
- To adhere to strict Infection Control in line with guidelines and policies.
- Advise patients and relatives/carers on general health care and ailments with referral to GPs as necessary.
- Experience in Primary Care or community nursing
- Able to work autonomously
- Registered Nurse (RGN)
- NMC Registration
- Experience in managing long-term conditions, particularly asthma
- Insight and understanding of current issues in nursing and NHS
- Excellent verbal communication & interpersonal skills
- Ability to work with staff of all levels in a positive, helpful and constructive manner.
- Ability to cope with change
- Experience of working in a GP practice
- Using clinical systems - EMIS or similar
