Discharge Liaison Officer

We are seeking a well‑motivated and committed individual to work within ward E54 & E55. The successful applicant will be part of the E54 or E55 nursing team and will help assistants nurses to plan timely discharge for all inpatients from point of admission to discharge to their own homes or alternative care facilities. In addition to this, the successful candidate will be responsible for managing patient records and other general administrative tasks.

Applicants will have previous clerical experience and excellent communication and organisational skills. It is essential that the successful candidate has the ability to organise and prioritise their own workload and achieve deadlines.

Ward E54 is a 30‑bedded ward and Ward E55 is a 31‑bedded ward which specialises in caring for patients who are admitted with respiratory and general medical conditions. In this role you will be working under the supervision of staff nurses and facilitate safe patient discharge home. You will need to have excellent communication skills to ensure all stakeholders are being informed and involved in patient discharge.

The post holder will be working Monday – Friday 08:00 – 16:00.

Main duties of the job
  • Assist RNs to plan and facilitate timely discharge for all inpatients, from point of admission to discharge to their own homes or alternative care facilities.
  • Effectively communicate information to patients, relatives, carers and all members of the internal and external MDT.
  • Ensure patients, their carers and members of the MDT are fully involved at all stages of the admission/discharge process.
  • Document clear and accurate information of discharge plans in the patient electronic record.
  • Collect and collate relevant performance information relating to discharge and length of stay (LOS).
  • Comply with the Trust’s vision, values and behavioural compact.
  • Undertake general reception duties along with other administrative support required for the day‑to‑day running of the ward.
  • Deal with enquiries from clerical and secretarial staff, medical and nursing staff, other Trust personnel and general enquiries from patients and members of the public, both face‑to‑face and over the phone.
