All screens Nationwide Ltd in Sunderland is recruiting a PCN Clinical Pharmacist to support medicines use through shared decision-making with patients, and to provide technical and administrative support to clinicians within the PCN.
You will undertake patient-facing medicines optimisation tasks, undertake medication reviews and reconciliations, and assist with ensuring safe prescribing, including EPS/Electronic Repeat Dispensing and online ordering.
