All screens Nationwide Ltd seeks a Lead Clinical Pharmacist for maternity cover to provide clinical leadership in medicines optimisation and safe prescribing within the practice setting. You will review discharge information, manage medication changes, and support prescribing safety initiatives.
You will liaise with PCNs, community pharmacies and secondary care, develop systems to improve medicines safety, and train GP teams on appropriate prescribing.
