Job summary

This is a part time post of 30 hours per week.

We are looking to recruit a highly motivated, enthusiastic, caring and compassionate nurse to a band 5 post within the Hospice Daye Services, at St Benedicts Hospice. The unit is part of an integrated Specialist Palliative Care Team that provide high quality palliative support to patients who have been diagnosed with a life limiting disease. The unit offers both physical and psychological support to patients and their families through MDT working.

The post holder will work as a part of a multi-professional team, providing skilled palliative care to a group of patients that visit the unit on a daily basis, providing comprehensive holistic assessments, emotional and physical support plans. Also providing patients with the opportunity to achieve their goals, whilst identifying and supporting family and carers.

Main duties of the job

The post will be a five day working week Monday to Friday, The working day is .

Aims of the service
  • To provide symptom, psychological and social/carer support on a weekly basis over a set period of time within a secure welcoming environment.
  • The patients receive a Nursing assessment on attendance, this may also require further referral or investigation into current health care plans.
  • The unit is multi-professional and is patient focused upon their current needs, each day the patients that are attending are discussed and implementation of intervention that may be required.
Roles and responsibilities
  • Holistic care needs assessment, including symptom and psychological needs.
  • Engaging with patients on a 1:1 basis
  • Facilitate group work/sessions to empower, engage and educate patients to improve wellbeing.
  • Support Patients and families with future care planning
  • Develop, implement and evaluate programmes of care.
  • Active participation in MDT meetings
  • Supporting and developing junior members of the team.

You must have outstanding communication skills, to work alongside complex situations and to be able to engage with patients, families, carers and other health care professionals.

Qualifications

  • Current NMC Registration (Level 1 adult nurse) or evidence of qualification if newly qualified.
  • Degree in Nursing or evidence of working towards.
  • Mentorship preparation.
