We are looking for a highly motivated Nurse Practitioner to work within the Sunderland Community Frailty Team in Sunderland. The team consists of Band 7 and Band 6 Nurse Practitioners who arebased in each of our six Community Integrated Teams. You will be responsible for out of hospital care assessment and co-ordination of care and services topeople who live in their own home or nursing or residential care and are identified as having moderate to severe frailty, multiple co-morbidities complex needs or who are identified as high intensity service users.

You will work as a member of the Multidisciplinary Team and will be required to attend and participate care homeand community MDTs. You will be an autonomous practitioner.

You will be responsible for pro-actively case finding patients with moderateto severe frailty and undertaking CGAs Comprehensive assessment, personalisedcare planning, providing support and advice to enhance self-care and improve quality of life for older people and reducing level of frailty wherever possible.

You will work autonomously, assessing individual patient needs, initiating investigations, with support determining adifferential diagnosis and initiating appropriate holistic, evidence-based treatment and care. You will refer patients who require medical intervention tothe appropriate services.

As a Junior Nurse Practitioner in the Community Frailty Team you will be an experienced Registered Nurse, whopreferably has experience at Band 6 level, you will have a great working knowledge of care of older people and patients with frailty and complex health needs and you will be a confident autonomous professional. You will be adynamic, approachable and self-motivated individual, youwill be responsible for ensuring high standards are maintained and you will havea desire for continual improvement.

You will have:

A full UK driving licence and access to a car

Registered Nurse with NMC

A nursing or related degree

Advancedclinical assessment skills accreditation or willingness to work towards

Non-medicalprescribing qualification or willing to work towards extended prescribingskills

Advanced decision-making skills
