COMPENSATION RANGE

$56.95 – $78.30/Hour (estimate) may be adjusted as permitted by law.

POSITION SUMMARY

The Shared Services After Hours Visit RN is responsible for providing skilled nursing visits to patients and their families as requested by Intella Triage. Documents observations and interventions related to the patient’s environment, physical, mental, emotional and social status. Coordinates care with other members of the Interdisciplinary Group (IDG) or team, including the patient’s physician.

AREAS OF RESPONSIBILITY
  • Admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs. Develops an individualized plan of care based on the assessment.
  • Obtains informed consent from patients and families and assists in completing forms required for hospice admission per regulations.
  • Assesses patient, provides and documents appropriate skilled nursing care, including teaching, family support and other actions as needed.
  • Assesses patient’s environment; physical mental and social status and eligibility for services in accordance with third‑party payee regulations.
  • Coordinates care with the patient’s case manager, informing him/her of significant changes in the patient’s condition and needs. Documents these communications.
  • Contacts patient’s physician when necessary.
  • Demonstrates all skills of a registered nurse, including vein puncture and management of high‑interventions in the home, e.g. IVs.
  • Maintains up‑to‑date and adequate stock of medical supplies and agency forms.
  • Knowledgeably discusses overall history, philosophy and services of Pathways Home Health and Hospice.
  • Provides patients and their families with guidance regarding health‑care decision‑making and options for making advance directives in coordination with the social worker.
  • Assesses all aspects of the patient’s pain and other symptoms and develops an individualized symptom‑management plan.
  • Anticipates, prevents and treats undesirable symptoms or secondary symptoms including use of non‑pharmacologic measures.
  • Supports, instructs and educates the patient, family, caregivers, and facility staff.
  • Documents nursing assessments, problems, appropriate goals, care interventions, patient and family outcomes, and evaluation of these in the electronic record.
  • Accesses electronic device during patient visit and documents during or immediately after the visit or per regulation.
  • Reports regularly to Shared Services After Hours Triage RN, as appropriate, and RN Case Manager or designee regarding care provided, and when patient condition is found to be unstable and there is a need for changes in current plan of care.
  • Attends educational in‑service trainings, and other agency meetings as requested by manager.
  • Maintains the dignity of the dying patient and supports the patient’s and family’s unique spiritual and cultural beliefs.
  • Provides holistic, family‑centered care across the treatment settings to improve the quality of life.
  • Knowledgeably discusses and initiates non‑pharmacologic interventions as appropriate to promote comfort, improve quality of life, and help manage emotional, spiritual, and physical distress.
  • Participates in the hospice program’s quality assessment performance improvement program and in‑service education related to areas of practice as appropriate.
  • Meets productivity standards as established by the manager.
  • When in a facility, adheres to “Pathways Standards of Care and Practice in Facilities” or equivalent document.
