Sunnyvale Gardens Post Acute is seeking a compassionate, organized, and dedicated Social Service Assistant to join our team. In this role, you will work closely with our Social Services Director to support our residents' emotional, social, and psychological well-being. If you are passionate about patient advocacy and helping individuals navigate post-acute care, we want to hear from you!
Compensation & Benefits Pay Range: $20.00 – $24.00 per hour (depending on experience)
Job Type: Full-Time Benefits:

Key Responsibilities Resident Advocacy:
Assist residents and their families with admission, orientation, and adjustment to the facility.
Discharge Planning:
Help coordinate safe discharge plans, including arranging home health care, medical equipment, or community resources.
Documentation:
Maintain accurate, up-to-date social service progress notes and assessments in compliance with federal and state regulations.
Support Services:
Identify and resolve resident psychosocial needs, acting as a liaison between residents, families, and clinical staff.
Room Visits:
Conduct regular one-on-one visits with residents to ensure their comfort, preferences, and rights are being respected.
Qualifications & Skills
Education:
High school diploma or equivalent required (Associate’s or Bachelor’s degree in Social Work, Psychology, or Human Services preferred).
Experience:
Minimum of 1 year of experience in a skilled nursing facility, post-acute care, or assisted living environment is highly preferred.
Communication:
Excellent interpersonal skills with a strong sense of empathy and patience for the elderly and medically fragile populations.
Organization:
Strong administrative skills, computer literacy, and the ability to multitask in a fast-paced healthcare environment.
