About the Role

We are seeking a compassionate, dedicated Registered Nurse to provide high-quality in-home care to our patients. As a Home Health Nurse, you'll deliver individualized care, support patient independence, and build meaningful relationships with those you serve.

Responsibilities

* Conduct in-home assessments and develop patient-centered care plans

* Provide skilled nursing care (medication management, wound care, monitoring chronic conditions, etc.)

* Educate patients and families on disease management and safety

* Document care accurately and timely according to agency standards

* Collaborate with an interdisciplinary team to coordinate care

Qualifications

* Active RN license in Kansas

* Home health experience preferred but not required (training available)

* Strong communication and clinical assessment skills

* Reliable transportation and valid driver's license

* Compassion, professionalism, and a commitment to quality care
