Responsibilities:- plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
- participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
- maintain treatment records, patient files, and progress notes as required
- participate in the development and implementation of patient assessments and care plans, including quarterly reviews
- current license as a physical therapist in the State of Wisconsin
