Position Overview
Hospice Promise is Nurse Owned. Our entire team is committed to making the end‑of‑life process with individualized care as well as a holistic approach.
Hours: Monday‑Friday 8 a.m.–5 p.m.
Benefits: Medical/Dental/Vision/Life
Vacation: 80 hours per year (prorated in first year)
AZ Sick Time: 40 hours per year
- Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs, providing a complete physical assessment and history of current and previous illness(es).
- Provides professional nursing care by utilizing all elements of the nursing process.
- Assesses and evaluates patient’s status, writes and initiates plans of care, regularly re‑evaluates patient and family/caregiver needs, revises the plan of care as necessary, and initiates appropriate preventive and rehabilitative nursing procedures.
- Determines nursing diagnosis using health assessment data.
- Develops a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions; includes the patient and the family in the planning process.
- Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
- Counsels the patient and family to meet nursing and related needs.
- Provides health‑care instructions to the patient as appropriate per assessment and plan.
- Assists the patient with activities of daily living and facilitates the patient’s efforts toward self‑sufficiency and optional comfort care.
- Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
- Operates as a driving position, must be willing and able to drive to patients’ places of residence.
- Bachelor’s degree from an accredited school of nursing.
- One (1) to two (2) years of recent acute care experience in an institutional setting.
- One (1) to two (2) years of recent experience in home care.
- One (1) year of hospice care experience preferred.
- Current licensure in State and CPR certification.
- Responsible for supervising hospice aides.
- Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills.
- Must be able to walk and stand for long periods of time; able to lift, position and/or transfer patients; lift supplies and equipment; reach, stoop, bend, kneel and/or crouch as needed; adequate visual acuity and hearing to perform required nursing skills.
- Personal car for travel and valid driver’s license.
- Carry personal auto liability insurance coverage.
