As a valued member of our healthcare team, your responsibilities will include developing tailored educational plans and conducting self-management training. You’ll be involved in assessing the effectiveness of educational programs and maintaining accurate patient documentation. Your work will contribute to continuous quality improvement and educational content development.

Key Responsibilities:

• Collaborate on educational goals for diabetes training

• Deliver comprehensive self-management training

• Assess patient progress with follow-up plans

• Maintain quality documentation and program effectiveness

• Update educational materials as needed

Requirements:

• Bachelor’s Degree in Nursing is essential

• At least 1 year in nursing or dietetics required

• CDCES credential necessary within 24 months

• Solid computer skills in Microsoft Office suite

• Capable of effective patient communication
