Description

The Prevention & Health Promotion program is responsible for the coordination and operation of primary prevention programs in Elementary, Middle and Secondary Schools. The goal is to provide meaningful and accurate information to youth, parents, caregivers and community partners to help youth make healthy lifestyle choices and decisions about substance use. Services are provided regionally in Tsawwassen, Ladner, North Delta, Surrey, Langley, Aldergrove and Chilliwack.

Position Summary

PCRS seeks a Prevention and Health Promotion Facilitator to work as part of a Prevention and Health Promotion team within the Delta, Ladner and Tsawwassen communities. The Facilitator is responsible for coordination and operation of primary prevention programs for children and youth using a broad-based health promotion approach. The Facilitator helps create healthy school and community environments and encourages youth to value healthy lifestyle activities.

Responsibilities
  • Screens, processes and accepts referrals, completes intake assessments and orientation as per policy, develops service goals in collaboration with youth and referring worker
  • Provides referrals and follow up for youth to appropriate community services
  • Acts as a positive role model and assists youth in developing values of honesty, self-assertiveness, and respect for others and lack of dependency on drugs
  • Offers presentations to contracted schools and community partners on a variety of topics related to substances, health and wellness
  • Develops, implements and facilitates group sessions; sessions could include co-leadership with other staff or community professionals
  • Collaborates with the contracted schools and district to provide workshops, large health promotion events and after school programming as determined by community needs
  • Plans and participates in pro-social, healthy recreation activities with youth
  • Participates in and attends working groups, committees, agency and school district meetings
  • Works collaboratively on large Prevention & Health Promotion initiatives with the P&HP team. These may include day camps, overnight summer camps, conferences and other group projects.
  • As per policy and agency practice, collects, documents and maintains statistical information to report to team lead and funders as required
  • Engages in supervisory meetings with team lead and reports critical incidents immediately
  • Maintains health and safety of self, participants and community; manages inappropriate behaviour according to agency policy and practice
  • Attends all agency, team and school staff meetings and professional development opportunities
  • Specialized training/experience in health promotion or community development
  • Knowledge and training pertaining to mental health, suicide and self harm
  • Knowledge of trauma informed practice
  • Other certifications in recreation or arts
  • Sound judgement and effective interpersonal skills
  • Clear and concise communication skills, both verbal and written
  • Substance use training
  • Ability to liaise with school staff
  • Familiar with working in a school setting
Qualifications
  • Education:
    • Required:

      2-Year Diploma or better in Child and Youth Care or related field.
    • Preferred:
      Bachelor's Degree or better in Child and Youth Care or related field.
  • Experience:
    • Required:

      Workshop development and presentation experience.
    • Required (1 year):
      Direct experience working with youth, with experience in schools an asset.
