Job Summary

We are currently hiring for a Relief Part Time Patient Care Coordinator, Pediatric Emergency to join our Pediatric Emergency Unit team at Surrey Memorial Hospital located in Surrey, B.C. This position is available until approximately September 2029 or until the return of the incumbent

Detailed Overview

The Patient Care Coordinator provides clinical supervision, guidance and support to nursing staff, other clinical team members and support staff within the assigned unit(s). The Coordinator has responsibility for the day-to-day operations of the assigned unit(s) including daily patient flow. Supports and participates in research, policy, quality improvement and risk management activities. The Coordinator performs duties with a unit or hospital focus and supports program and/or regional activities.

Responsibilities
  • Apply established criteria and prioritize admissions/intake, lead and facilitate actions related to daily patient flow, plan for patient discharge and appropriate patient placement/referral by ensuring the care team follows through on specific care paths and processes and by reviewing the admission/intake and discharge planning of patients and individual care plans to ensure optimal progression to discharge, in collaboration with other clinical team members.
  • Supervise nursing and nursing support staff and coordinate the work of the clinical team overall on a day-to-day basis; observe work and correct where necessary, monitor quality of care and patient safety, assign and adjust work assignments, schedule and coordinate staff, monitor and may authorize staff hours of work, including overtime, vacations, leave requests, ensuring service levels and staffing requirements are met.
  • Support quality patient care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out patient/family education regarding illness prevention and health maintenance, acting as a mentor and resource to staff, role modeling best clinical practices, assessing clinical skills and by coaching staff and/or liaising with other health professional leaders to help them achieve established clinical performance goals.
  • Initiate/lead/support plan of care conferences and rounds (e.g. iCare) for the purpose of optimal patient care management according to best practice; disseminate clinical information to nursing/nursing support staff and other members of the clinical team.
  • Identify learning requirements of staff and, in conjunction with the Manager, Educator and other members of the clinical team, provide input into the development of learning objectives; provide clinical support and guidance and evaluate the effectiveness of learning opportunities.
  • Provide input into the development of a comprehensive orientation program, in collaboration with the Educator and other clinical staff.
  • Act as a point person for patients and families who have concerns or questions regarding their care; follow up as required; participate or lead investigation of complaints by patients/families; follow up on complaints and implement corrective action as appropriate; collaborate with Quality Improvement according to established protocols.
  • Participate in quality improvement and risk management activities by methods such as evaluating effect of changes in practice, generating recommendations for alternative approaches to practices, conducting safety audits, and supporting implementation of teaching/learning when required.
  • Assist the Manager/Director by attending panel interviews, providing recommendations on the selection of new staff, providing feedback to the Manager on staff performance appraisals, and providing feedback on optimal use of budgetary resources and regarding minor and capital equipment needs as requested.
  • Promote staff awareness and involvement in research/evaluation activities and special projects and participate in research and special projects as appropriate.
  • Collaborate with internal and external stakeholders, participate on a variety of designated professional or operational committees at the unit, hospital, program and/or regional level.
  • Participate in the review, development, implementation and evaluation of evidence-informed clinical decision support tools/policies.
  • Perform other related duties as assigned.
Qualifications

Education and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent…
