Clinical Pharmacist - Surrey Memorial Hospital;
Fraser Health Authority. Full-time opportunity based in Surrey and Langley, BC, with primary work at Surrey Memorial Hospital and time as needed at Langley Memorial Hospital.

Salary: CAD $51.37 - $64.19 per hour.

Are you looking for a place to showcase your clinical hospital pharmaceutical skills? The successful candidate will provide clinical pharmacy services and support for SMH and LMH, with the majority of time spent at SMH.

• Provide individualized pharmacotherapy and counsel patients about their medication therapy.
• Collaborate with physicians and other health care providers to develop, implement and monitor therapeutic plans.
• Assist in developing drug therapy guidelines for disease conditions.
• Prepare and submit case studies and articles for publication; act as department resource within your assigned area.
• Participate in medication use evaluations, quality improvement initiatives, and clinical drug trials.
• Mentor and supervise pharmacy students, residents and new staff.
• Ensure prescriptions are validated and dispensed accurately; address medication safety concerns with the health care team and document professional practice activities.

Education and Experience
Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience.

Skills and Abilities
Broad knowledge of organizational policies and standards of care; evidence-based clinical pharmacy practice; medication use evaluation; research processes; hospital drug distribution; ability to apply clinical knowledge with minimal supervision; effective communication; ability to work with diverse teams; ability to organize and prioritize work; physical ability to perform duties.
