Clinical Pharmacy Specialist - Jim Pattison Outpatient Care & Surgery Centre — Surrey, BC
Salary: CAD $53.31 - $66.63 / hour
We are seeking a Full Time Clinical Pharmacy Specialist to join our team at Jim Pattison Outpatient Care & Surgery Centre, located in Surrey, BC. Fraser Health Authority partners with Provincial Health Services, Providence and Vancouver Coastal Health to provide integrated pharmaceutical care and service excellence.
Responsibilities- Provide consultation services to internal and external health care professionals and apply advanced theory to assess, plan, implement and evaluate the delivery of pharmaceutical care in the designated specialty area.
- Provide clinical direction on complex cases using evidence-based evaluations to assess drug therapies and patient medical histories; attend patient care rounds; review medication histories; provide drug information, patient education and discharge consultations; and provide recommendations in policies and procedures.
- Ensure quality assurance programs for the designated specialty area, identify and troubleshoot inconsistencies, develop standards, conduct regular audits and take corrective action; participate in quality improvement activities; prepare reports as required.
- Provide academic teaching to undergraduate and postgraduate students and contribute to hospital-based pharmacy programs in cooperation with the Faculty of Pharmaceutical Sciences, University of British Columbia.
- Identify and initiate clinical drug research, including data collection and analysis, and write articles for submission to scientific journals.
- Conduct pharmacological education or in-service sessions; participate in Pharmacy and Therapeutics Committee activities; contribute to formulary decisions and policy-making.
- Collaborate with program team to develop, implement and evaluate goals, objectives and outcomes for the designated specialty area.
- Represent the department on multidisciplinary care committees and at external events such as conferences and regional planning groups.
- Provide pharmaceutical care to patients in the designated specialty area; monitor therapy and outcomes; educate patients and families about drug status; identify and solve drug-related problems.
- Perform other related duties as assigned.
Education and Experience
- Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
- Completion of year 1 and year 2 of a Hospital Pharmacy Residency Program accredited by the Canadian Pharmacy Residency Board (CPRB), supplemented by two years' recent, relevant hospital pharmacy experience including one year in the designated specialty area or an equivalent combination of education, training and experience.

- Ability to establish curricula and conduct performance evaluations for clinical rotations within the assigned area.
- Ability to independently recommend, implement, monitor and alter pharmacotherapeutic plans based on patient-specific information.
- Ability to use focused physical assessment to evaluate efficacy and/or toxicity of drugs.
- Ability to retrieve and evaluate drug literature to support clinical care pathways and medication use evaluations.
- Ability to plan, lead and coordinate project teams, pharmacists and students.
- Ability to conduct clinical research studies and publish findings.
- Ability to establish workload priorities in collaboration with others.
- Ability to operate related equipment and perform the physical duties of the position.
