In this position, you will independently assess and manage client caseloads while collaborating with various healthcare professionals. Key Responsibilities:
• Establish supportive relationships with clients and families
• Coordinate and review client care plans
• Deliver direct nursing care and health education
• Collaborate with multidisciplinary teams
• Maintain comprehensive clinical documentation
Requirements:
• Current registration with BC College of Nurses
• At least 1-year recent clinical experience in similar settings
• Valid BC Driver's license and access to a vehicle
• Knowledge of chronic illness management and community resources
• Ability to adapt to changing client needs and priorities
