As an Outreach Worker, you will collaborate with a dedicated health team to assist clients in accessing opioid agonist therapy and related services.
Your role involves monitoring client progress, arranging transportation, and linking clients to necessary community programs. With a focus on advocacy and crisis intervention, you will play a vital role in promoting well-being.
Key Responsibilities:
• Monitor health conditions and document observations
• Plan and escort clients to critical appointments
• Assist clients in accessing benefits and resources
• Administer medications as prescribed by the Registered Nurse
• Provide insights on health promotion and life skills
Requirements:
• Completion of Substance Use Certificate and relevant experience
• One year experience with substance use issues
• Valid BC driver's license
• Current CPR training certificate
• Strong communication and crisis intervention skills
