Mental Health Outreach Worker

This role focuses on assisting clients within the Supported Living program. As a key team member, you will assess psychosocial needs, help clients achieve independence, and support them in daily tasks like budgeting and cooking. You will collaborate with professionals and maintain thorough progress documentation to promote client well-being.

Key Responsibilities:

• Assess clients' psychosocial rehabilitation needs

• Provide support for daily living activities and community integration

• Facilitate education, training, and useful resources

• Maintain progress notes and collaborate with other professionals

• Respond to mental health emergencies as necessary

Requirements:

• Mental Health Diploma or Certificate; degree in social services preferred

• Minimum one year of experience with vulnerable populations

• Current Level 1 First Aid certificate required

• Unrestricted Class 5 Driver’s License and reliable vehicle needed

• Strong understanding of psychosocial rehabilitation practices
