In this role, you will cater to the physical, intellectual, and emotional support requirements of patients through personalized care strategies. Candidates must have completed a recognized PSW Program or hold HCA/CCA certification. Experience in providing care for dementia or palliative patients is advantageous and will enhance the quality of support provided.
Key Responsibilities:
• Provide direct personal support including bathing and grooming
• Follow care plans to meet patient needs effectively
• Communicate positively with patients and their families
• Document care activities according to policies
• Support patients in daily activities for improved quality of life
Requirements:
• Completion of PSW Program or HCA/CCA accreditation
• Experience in home care or long-term care settings
• Knowledge of care documentation and planning
• Proficient in English reading and writing
• Capability to work independently in community care scenarios
Deliver dedicated care and compassion to enhance patients’ well-being in community settings.
