The Clinical Social Worker will aid in both Urgent and Primary Care settings, conducting thorough social work assessments and facilitating access to community resources. By collaborating with healthcare teams, you’ll improve client services while providing trauma-informed, culturally safe support to patients and their families.

Key Responsibilities:

• Conduct psychosocial assessments to inform treatment decisions

• Collaborate with teams to develop patient-centered care plans

• Offer brief therapeutic interventions for mental health issues

• Maintain accurate patient records and documentation

• Participate in local committees to enhance service delivery

Requirements:

• Current registration with the BC College of Social Workers

• Minimum two years of related experience in health care

• Valid BC Driver’s license and access to a vehicle

• In-depth knowledge of counseling models and community resources

• Effective communicator with strong organizational skills
