Detailed OverviewReporting clinically to the Professional Practice Chief, Social Work and administratively to the Manager, Clinical Operations, the Social Worker provides comprehensive social work services and therapeutic clinical counseling to patients and families in the assigned clinical specialty area. Responsibilities include conducting comprehensive social work and psychosocial assessments, delivering grief and bereavement counseling, providing consultation and education to patients, families and health professionals, maintaining accurate records, leading interdisciplinary care planning, coordinating community resources, participating in local committees, developing and delivering in‑services, and initiating research activities relevant to patient and community needs.
Responsibilities- Conduct comprehensive social work and psychosocial assessments of patients and families by interviewing, gathering social data, and interpreting information using a variety of tools in accordance with professional standards and clinical policies.
- Provide therapeutic clinical services such as grief and bereavement counseling, exploring emotional responses and facilitating coping strategies, coordinating services, and managing case meetings.
- Provide consultation and education to patients, families and other health professionals on issues related to high‑risk factors, recovery, and current practice in adaptation, crisis management, protection of vulnerable adults, chemical dependency, and community support services.
- Maintain patient records by documenting assessments, charting information, and preparing progress notes and reports in compliance with established standards and regulatory requirements.
- Lead interdisciplinary team in patient care planning, including discharge planning, by assessing care needs and articulating decisions to team members.
- Arrange admission meetings to coordinate care plans and community services for patients requiring facility placement or home support, collaborating with hospitals and community services.
- Communicate with community health and social agencies, ministries and family physicians to manage referrals, share information and ensure appropriate long‑term follow‑up.
- Act as a clinical resource to patients and families, providing information on community resources, facilitating referrals, and developing community support.
- Participate as a member of planning bodies, evaluating programs, identifying opportunities for change, analyzing outcomes, and improving service delivery and effectiveness.
- Maintain workload statistics, analyze trends, account for variances, and prepare reports or action plans as required by the Manager, Clinical Operations.
- Develop, coordinate and deliver in‑services, presentations, psycho‑educational sessions, and support groups for the interdisciplinary team, including planning, resource acquisition, and literature searches.
- Initiate and carry out research activities, program planning and development, write proposals, monitor budgets, conduct methodology, disseminate findings, and ensure protocols are consistent with research.
- Maintain professional development by reviewing literature, consulting experts, evaluating clinical practice and engaging in professional activities to improve care.
- Perform other related duties as assigned.
Education and Experience
Master’s degree in Social Work from an approved school of Social Work and one year recent, related acute care clinical experience. Current full registration with the BC College of Social Workers.
