Fraser Health seeks a Social Worker committed to mental health and community resources. You will work within a multidisciplinary team and engage in psycho-social assessments to guide client treatment plans. This role includes hands-on support in mental health, collaboration with healthcare professionals, and delivering specialized educational groups.
Key Responsibilities:
• Conduct psycho-social assessments and create treatment plans
• Collaborate with primary care professionals and community resources
• Organize psycho-educational groups for clients and families
• Facilitate client access to community services and rehabilitation
• Maintain documentation and advocate for client needs
Requirements:
• Degree in Social Work and current registration with BC College
• Minimum two years' relevant experience in mental health
• Valid Class 5 BC Driver's License and access to a vehicle
• Comprehensive knowledge of psychiatric disorders and assessments
• Ability to communicate effectively with diverse populations
