Key Responsibilities:

• Provide advanced psychosocial assessments and referrals

• Coordinate care plans tailored to client needs

• Engage with community service agencies for client support

• Conduct clinical consultations with the nursing team

• Participate in inter-professional care planning
Requirements:

• Bachelor's degree in Social Work

• At least two years of related experience

• Valid B.C. driver’s license

• Strong skills in community collaboration

• Ability to handle complex assessments and interventions
