Clinical Social Worker Position

Fraser Health is seeking a Full-Time Clinical Social Worker specializing in Toxic Drug Response and Priority Populations in Surrey, B.C. This position focuses on delivering comprehensive care and clinical support in a collaborative environment.

You will join a dedicated team to conduct psychosocial assessments and provide therapeutic services to families and clients. Collaborating closely with community providers, you will facilitate impactful care plans and ensure continual support for those at risk.

Key Responsibilities:

• Conduct in-depth psychosocial and social work assessments

• Provide guidance and support to clients and families

• Collaborate on interdisciplinary care plans

• Document assessments and maintain patient records

• Initiate community partnerships for resource allocation

Requirements:

• Current registration with the BC College of Social Workers

• Bachelor’s degree in Social Work from an accredited program

• Two years of experience in a complex health care setting

• Access to a personal vehicle with a valid BC Driver's License

• Demonstrated ability to communicate effectively with diverse groups
