Detailed Overview
In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively and in partnership with the interprofessional care team in the provision of person-centered care, the Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care to patients in an acute medical unit.
Responsibilities
- Assesses, plans, implements and evaluates nursing care of assigned patients. Identifies actual or potential problems, plans interventions, performs planned interventions and evaluates outcomes. Collects information from a variety of sources using skills of observation, communication, data review and physical assessment.
- Coordinates and provides nursing care to assigned patients according to physician and anaesthetist orders and in conformance with established standards and hospital policies. Sets priorities, assigns and delegates tasks to appropriate staff.
- Modifies care as needs change. Initiates immediate treatment, such as applying non-invasive monitoring devices if required.
- Explains procedures to the patient and/or teaches the patient how to deal with the result of the procedure.
- Obtains, checks, sets up and positions supplies, instruments and equipment required for interventional procedure ensuring that instruments and equipment are in working order. Reports instruments/equipment in need of repair.
- Performs circulation duties including passing instruments and supplies, medications and other materials to the team.
- Documents assessments, nursing diagnosis, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Collaborates with members of the inter professional care team including Physicians, Licensed Practical Nurses, Patient Care Coordinator, Clinical Nurse Educators, and other health care professionals/providers in the identification and resolution of issues, adjustments to care plans as required, and in regard to changes in patient condition.
