Job SummaryBring your clinical expertise and commitment to compassionate, high quality care to a dynamic and meaningful role as a Locum Nurse Practitioner with Fraser Health. We’re seeking an experienced and empathetic NP who is ready to make a real impact across multiple sites and communities.
As a Locum NP, you’ll be a key contributor to inpatient units and Patient Assessment and Transition to Home (PATH) units, with the flexibility to support additional settings such as primary care clinics. Your presence helps ensure seamless care delivery, strengthens community health, and supports patients and families through pivotal moments in their care journey.
The PATH program is a hospital‑based inpatient service where Nurse Practitioners assume the role of Most Responsible Provider (MRP). Working with medically stable, alternative level of care (ALC) patients who have complex discharge needs, NPs in PATH empower individuals to transition safely and confidently back into the community.

- Application of geriatric care principles to support a defined patient population.
- Collaboration within a specialized, team‑based environment focused on reducing hospitalizations, preventing health deterioration, and improving quality of life.
- Effective referral, consultation, and navigation of community resources to support safe transitions home.
