Patient Care Coordinator - Mental Health

This role requires a registered nurse or psychiatric nurse with at least three years of clinical experience, including one year in a supervisory capacity. As a key member of the Mental Health and Substance Use Services team, you'll oversee daily operations, enhance patient care quality, and mentor nursing staff. Your leadership will ensure effective patient flow and collaboration with clinical teams to implement best practices.
Key Responsibilities:
• Prioritize patient admissions and manage discharge planning
• Supervise nursing staff and coordinate daily clinical operations
• Facilitate team collaboration and mentor staff on best practices
• Conduct care rounds and organize patient care conferences
• Support quality improvement and risk management initiatives
Requirements:
• Registration with BCCNM as a Nurse or Psychiatric Nurse
• Minimum three years clinical experience, one year in supervision
• Valid Class 5 Driver's License and access to a vehicle
• Strong leadership and communication skills
• Proficiency in clinical practices and team management
