Detailed OverviewIn accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively and in partnership with the interprofessional care team in providing person‑centered care, the Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care to patients in an acute medical unit.
Responsibilities- Assess patients, collect information from a variety of sources using observation, communication, data review and physical assessment; make a nursing diagnosis; plan and evaluate nursing interventions and care.
- Collaborate with patients and their families/caretakers to develop the care plan, encourage family participation, establish therapeutic relationships, advocate for patient rights, and ensure patient choice.
- Participate in care and discharge planning on admission, ensuring effective implementation, updating the plan, collaborating with the interprofessional team and communicating with the Patient Care Coordinator on issues affecting discharge.
- Coordinate nursing care, set priorities, assign and delegate tasks to support staff, and refer to the interprofessional team according to protocols.
- Receive and prepare patients for testing, review requisitions, correlate history with examination, explain procedures, and monitor comfort.
- Perform non‑invasive diagnostic tests on the vascular system, including ankle‑brachial and toe‑brachial indexes, thoracic outlet testing, Doppler and plethysmography; perform TCOM, assess results, record findings, and submit to physicians.
- Collaborate with interprofessional team members to identify and resolve issues, adjust care plans, and respond to changes in patient condition.
- Teach patients and families about illness prevention, health restoration and maintenance; assess learning needs and provide instruction.
- Plan and discuss referral information with patient and families, collaborate with the team, and initiate referrals to appropriate health care/community agencies.
- Document assessments, nursing diagnoses, observations, interventions, responses, outcomes, and referrals according to standards.
