Detailed Overview
In accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM), and the Mission and Values of Fraser Health, the Community Health Nurse (CHN) - Licensed Practical Nurse works independently in the community setting. The role includes collaborating with an interprofessional team to manage an assigned client caseload, conduct assessments, coach, intervene, and provide client‑care services with the goal of enabling clients and their families to live confidently and safely at home or in the community.
Emphasis is placed on promoting, maintaining, and restoring health, teaching self‑management, and facilitating transitions across the healthcare continuum to minimize avoidable admissions to residential or acute care facilities. The nurse will ensure linkages with acute, primary, and community care providers, including physicians, nurse practitioners, and other specialists, as well as family and community support systems. Eligibility for the role is based on employment status.
Responsibilities
Establishes a therapeutic relationship with the client through interpersonal and interviewing techniques, in person and/or over the telephone, ensuring the client’s choice, autonomy, dignity, and privacy in decision‑making and care planning.Screens referrals, provides individualized client assessments, interprofessional care planning, and appropriate interventions. Assists clients with complex chronic conditions to achieve optimal function by coordinating timely health services and resources. Collaborates with primary care providers, clients, families, caregivers, and community resources to resolve care issues and integrate services.Initiates and develops comprehensive shared client care plans in collaboration with the interprofessional team, primary care provider, client and/or family, and other healthcare providers or referring clinics. Facilitates the transition of the care plan to the referring source and community agencies.Provides direct client care, identifies additional required services per guidelines and evidence‑based practice, and offers thorough explanations of care to the client and family.Where applicable, assigns direct client care tasks to Community Health Workers, provides demonstrations and training, and collaborates with the Community Health Nurse
- Registered Nurse to monitor tasks within defined limits.Facilitates care conferences to review client plans, determining timing, referral, and interventions to improve outcomes. Initiates, monitors, and evaluates care plans, developing next steps and a transition process across the continuum.Discusses clinical findings with the client/family, collaborates with the interprofessional team to develop action plans and goals focused on clinical needs, self‑management, and quality of life. Utilizes system information to review client data and incorporates findings into care plans, prioritizing resources efficiently and responding to unanticipated events.Supports clients and families before, during, and after conferences and clinic visits by providing information throughout decision‑making, advocating for client choices, and delivering direct care within clinic, community, or home settings.Arranges and participates in joint home visits with other healthcare professionals, advising clients and caregivers on available resources.Provides health counseling, education, self‑management training, and wellness promotion through clinic, telephone, or home visits.
