Join the Specialized Community Services Program and work with the Complex Medically Frail team. The Community Health Nurse operates independently while collaborating with various providers to ensure clients receive comprehensive care. You will conduct assessments and coach clients, providing essential services that help them live confidently at home.
Key Responsibilities:
• Establish therapeutic relationships with clients and families
• Conduct individualized assessments and care planning
• Develop shared care plans with interprofessional teams
• Provide direct client care and health counseling
• Manage clinical records and quality improvement activities
Requirements:
• Licensed Practical Nurse registration with BCCNM
• One year related clinical experience required
• Valid BC Driver’s license and vehicle access
• Knowledge in gerontology and chronic illness care
• Strong communication and teaching skills
